Please do not post your personal information on the community forum.
Right where you post is, click option on the right to edit your post.
Instead, you should get a hold of PM and provide your personal information thru the below methods:

and/or
Private message mod from here
I would recommend you to do both. Please take note that at the moment, there's probably going to be a week long wait for fixing this issue. You can also try message public mobile through facebook.I had better luck with faster response there.
