If you wish to obtain a new PM SIM card, the only method is by sending a web form to the company.

Look for Contact Us in the lower column on this site's homepage. Click that, and a new page will open with Email Away on it. Click that, and you will be brought to a web form. Complete it as appropriate. Remember to include the size of SIM card required.
