As chuk said, the self-serve account lacks the ability to create a PUK code.
If you need one, and became a customer prior to January 27, 2015, call *611 from your cell, or 1-855-4PUBLIC from any phone.
If you became a customer on or after January 27, 2015, you must communicate with Public Mobile by web mail only. Click Contact Us in the left column of the home page. A new page will open with Email Away on it. Click that, and another new page will open with a web form and a hyperlink called None of the above. Click None of the above, and submit it to Public Mobile.
