Can we have two different phone number under the same account, or we have to creat a separate account for each phone number?

You need to create an individual account for each phone number. You can active a new line first. If you port your number, you need to wait couple day. Then you can receive the referral reward,otherwise, it may not recognize your number as PM customers. Thanks

Also you have to use a different email for each account.
