For example, top 5%, 10%, 50%, will the corresponding reward

given once on the month that the user achieve one of the level,

or it will be always given once it's achieved,

or it will change every month depends on the user's current level?

It changes every month.

Once the calendar month is over, they make secret calculations about your participation in that given month, and the reward is added to your account around the 6th of the next month. You can see it added there on the main page of your selfserve:

The amount pointed by the arrow is then applied to your next billing cycle, and at this moment it will go back to $0 in the reward section of your selfserve account, until the next month 6th-ish, if you kept participating.

Other than Oracles, they all refresh each month. Everyone's on an equal starting point each month.
