I purchased the Sim card and I tried to switch my plan from koodo to public and I use my Sim card number. I wasn't able to sign up. I tried it again and my Sim card doesn't work anymore.

Any suggestions what can I do or who to contact to help me

Due ot Koodo data breach, some Koodo accounts have a port protection feature.  Did you call Koodo to unlock the feature?  

 

Also, check your credit card to make sure that you were not charged for the failed activation.  If charged, re-insert the PM Sim card to see whether it works.

And When entering your debit/ credit card information:
1. Enter the credit card address exactly as it appears on your statement ( capitalize all if necessary ).
2. Leave the apartment field blank, if applicable enter your apt in the street address field ie xxx - street address.
3. Enter your postal code without spaces ( if there is a space on the credit card statement then enter the first 5 digits only with the space).

try again after 1hr or so, or use incognito mode on your browser. make sure you are typing in the 19-digit SIM number correctly. maybe theres a typo thats why you are not able to activate the SIM card.

Is your Koodo account prepaid ?  If it is then you need to submit a ticket by clicking on the question mark on the bottom right.  Type want to speak to a human.
