When I dial 1 to setup my voicemail on my iphone 6, I get an 'Error Performing Request / Unknown Error' screen. My wife can call my number and receive a generic Voicemail message, but I cannot check my Voicemails or set up my greeting. Any advice?

You can try this:

No guarantee but it worked for me so maybe its worth a shot.

@DFletch Try calling your number from another phone, but interupt the greeting by pressing *. That normally leads to the setup menu.
If you were tranferring a phone number, does that SIM still work?

I tried resetting the phone - no luck. I tried calling my own number, and I just get silence and then 'call failed'.

Are you a bot? The first thing I said was that I get an error when dialing 1 to initiate the Voicemail setup. I have Talk minutes included in my plan.

It is likely still setup to call the voicemail of your previous provider. Try resetting/restarting your phone.
You can dial your own number or one of the voicemail mail numbers (such as area code + "580-4001". And optionally add this number to your speed dial.
