Hello,
If you be came a customer before January 27, 2015, you can call *611 from your phone (if it wasn't lost) or 1-855-4public from any phone.
If you became a customer after January 27, 2015, you must communicate with PM by web mail. Go to this site's homepage, and look for Order a SIM card and proceed from there.
Since your phone is lost, it would be best if you accessed your online account to place your phone in suspension by declaring it Lost or Stolen.
