Buying an addon has 2 important STEPS:
1-ensure you have added funds in your Available Funds area.
2-GO BACK to ADD(purchase) the addon to your account (it should then show up in your Overview area). This manual step is sometimes overlooked.
EDIT: Reboot phone after addon is completed.

*what is mean for Rebooting the phone means to turn off your phone and turn it back on again.
To get the obvious out of the way...you used ( +1 or 1 ) in front of the any area code,
for Example long distance in canada or from USA,, Add (+1 or 1 ) in front of the any area code,
1416xxxxxxx,
or
+1416xxxxxxx,
both is will going through your call...

@ellanne for your part about No incoming long distance, actually if you really meant you are not receiving incoming calls from your friends overseas, then it is a different issue. It has nothing to do with your long distance add-on. You dont need long distance add-on to receive overseas call.
My question to you would be.. did you ever receive call from the oversea person you are expecting ? it works before and just stopped working or it never works? check if your phone has any blocking setting that might have blocked the incoming long distance by mistake. Or you have enable blocking unknown callers and the long distance calls didn't come with proper caller id and hence treated as unknown caller? Also, if you have way to contact your long distance friends, confirm if they calls got to voicemail instead or not coming through at all.

Try putting your phone on 3G-only (WCDMA).
Also try to reseat your SIM and do a network reset.

Also, where you are calling? The Long Distance add-on offered by PM only works for calls to selected countries:
Intl long distance Add-On included countries: Australia, Austria, Bangladesh, Brazil, Chile, Colombia, Czech Republic, Denmark, France, Germany, Greece, Iceland, Ireland, Israel, Italy, Japan, South Korea, Malaysia, Mexico, Netherlands, New Zealand, Norway, Peru, Poland, Portugal, Puerto Rico, Romania, Singapore, Spain, Sweden, Thailand, Venezuela. Calls must originate from Canada.

Try rebooting your phone. If you have another then try swapping the sim. You may also try a network reset.
