I am in New Brunswick, physically, with my iPhone 14 Pro Max. I have been a PM customer for 7+ years.
For some reason, unknown to me, I have colleagues in the USA trying to call me and either get a fast busy signal, or get some odd (what I believe) carrier message stating to try again later. I have had callers on AT&T, Verizon, T-Mobile to mention a few .. so not just a single carrier.
This has been going on for 9+ months, maybe even over a year. Sure feels like something on this end?
Any thoughts as to what might be causing this?

My first thought here is that you might have some setting enabled on your phone to block or reject "unknown" callers, which might include callers from another country, unknown caller ID, etc.
To help see if this might be a cause, look for any blocked caller settings and turn them all off. Here is apple's article on how to configure those settings:
https://support.apple.com/en-ca/guide/iphone/iphe4b3f7823/ios
Secondly, if you are using a physical SIM, try moving the SIM card into another phone and see if the problem persists. Even try moving it to an Android phone instead of an iPhone if possible too...
Finally, it is interesting that their calls are not at least routing to your voicemail, which is normally where calls that are auto-declined by your phone should always end up... Just to ensure you have not inadvertently customized your conditional call forwards away from the defaults, try dialing both: ##21# and ##004#.

if I reboot my phone any more, it will melt 😉 I just reset my network settings now, and will try that
I can call them all just fine, zero issue. Yes I have a can/usa plan just as an FYI

@Luschka That is strange you should be able to get calls from anyone on earth , so I tend to think it has to do with them . Do you happen to have long distance or CAN/US plan ? Are you able to call them ? Can try to reboot your phone or reset network settings
but can also confirm with support
