You'll have to get that from a Customer Support Agent.

@likefree  you sure that is a sim card PUK it was asking for?

What was the exact error message? there are different PUK and need to act differently depends on the message

If the message is “PIN unlock key (PUK)” or "Enter PUK Your SIM cannot be accessed" or on an iPhone's status bar showing  "Locked SIM"
