Reboot without the Koodo physical SiM card. Then try the network reset of phone.
IF that does not work, you will need to contact customer service agent. However, CSA is not working right now and will respond by tomorrow morning.

Physical SIm is Koodo, I have answered yes to port request, so I have removed the physical SIM. Still no dice.

Sorry, I was confused when you mentioned the physical SIM card. If the physical SIM card is Koodo, if you already answered yes to port request, then remove the Koodo physical SIM card.

I had a PM number using an eSIM, and a Koodo number using a physical SIM. I was trying to migrate the Koodo number over to PM.

Just to clarify, you are trying to get two PM phone lines. One on eSIM (original PM number) and another on physical SIM card (ported number)?
Network reset will not erase data, contacts, etc. Just saved Wifi password and bluetooth settings. Depending on your phone (android versus apple), it should be under main settings. General settings for Apple.

Try a simple reboot of your phone (if not done already). If that does not work, then network reset of phone. (note: this will erase any saved Wifi passwords)

Initiated about 12 hours ago. Received port request from old carrier and replied yes, that was five hours ago. Already had an active PM eSIM for a different number. Now I have no service with either the migrated number or the old number.

How long was it since you initiated the port request? Did you receive the port request text from your old carrier? Did you respond yes? It can take 1-2 hours for mobile port requests. Landlines and VOIP phone numbers can take several days to port over.
Did you activate eSIM or physical SIM card? Do you have any service with PM sim card?
