In terms of length of time referral stays and discount is applied to monthly bill, does it stop after 2 years, for example?

On plan builder, there was never a 30-day available that was that expensive.
As for the maximum rewards on a 30-day plan, your understanding is correct, but it's actually possible to get a higher Community reward amount applied for a 30-day renewal if the Commmunity rewards were awarded too late during one month to be used for a renewal, but the next month's Community reward is issued before the following renewal.

Was it even possible to get a plan with such high cost through the old plan build calculator?

Please take a look through this thread. There is a cap, but that cap isn't the amount of your plan fee. It's actually quite a bit higher than that.

Yes, there is a cap. The refer-a-friend credit limit is $110 for a 30 day plan. You can refer more people than 110 people if you're on a 30-day plan, but you will never receive a refer-a-friend reward amount higher than that $110 amount. There is also a 90-day plan refe r-a-friend credit limit ($250), but that limit is less favorable when you divide by 3.

The topic being discussed in this thread is about something different. The offer that you're speaking of was offerred to some customers who have a a low amount of referrals to encourage them to start referring new customers. This thread is related to if there is aabsolutely a mamximum number of people or amount of discounts that can be received from the refer-a-friend program.

This has already been answered. There is a limit for refer-a-friend rewards. It is not unlimited.

Beside the 110 limit, you will continue to receive $1 per 30 days cycle as long as your referrals stay with PM.
