Over the past two weeks I have missed three important phone calls from work and the circumstances around these missed calls have me puzzled. They occurred on
The nature of these missed calls include
I am seeking advice because although the missed calls are the minority of cases, they have dire consequences in regards to my employment.
I have verified with my workplace IT department to ensure that there has not been any technical problems on their end that would produce the above symptoms.
I have also confirmed with Telus to ensure that there has not been any outages during the above date and times; in fact they reported no outages in Ontario for all of February.
I checked my usage on My Account today and there is actually record of the three missed calls above; they do not appear on my phone's call log, but Public Mobile seems to have a record of them. Interestingly however, these calls are listed as "Outgoing Calls", not incoming ones. I have attached screenshots.
So far, the issue doesn't seem to be arise from the caller's end. The Telus network seems to be working properly. My phone settings are confirmed to be appropriate. Public Mobile seems to receive and have record of the missed incoming calls but they are logged strangely. And my phone has no record of the missed incoming calls.
I restarted my phone between each of the three incidents.
Does it seem plausible that there may be a breakdown between Public Mobile after they receive the incoming calls and the calls getting to my phone?
Can you please advise if any technical problems on Public Mobile's behalf may have produced the symptoms I described above?
I'm not trying to point any fingers. I'm just trying to solve this mystery so I can reliably receive phone calls from my work.

Can you confirm again? Those numbers indicated are for voicemail retrievals.

They are not missed calls. They are outgoing calls and the numbers ending in 4001 are the numbers used to retrieve your voicemails.
