First, PM fails to honor my community rewards from April. Then, they start sending me surveys asking me why I left. Now, they even lock me out of my own account. I tried logging in as normal and gets denied. Tried changing my password, and gets denied again. Maybe this is their way of telling to GTFO. Is this even legal?

@CalvinW, WOW you are persona non grata with Public Mobile! I suppose all the pieces of the puzzle fit together nicely. Somehow you have been dropped off the active customer list. Thus, no reward credits, the survey email on why you left and being locked out of the account. I count 3 strikes here, so you are out.
Sorry for picking on your misery. Sometimes you gotta laugh a bit in the face of the adversity. It is comforting self medication. Hopefully they straighten things out and compensate your inconvenience.

Not sure if you already know this or if it applies in this instance, but you need to submit the same email address to your PM Self-Serve login and your PM Community login. If they're different then your Community Rewards won't be properly applied.
I've received "tell us why left" surveys a few times while obviously continuing to use my PM service uninterrupted. Quite random for me. So perhaps just a bad timing coincidence for you.

Many other users received the email and some experienced interruption in their service. We have reached out to the team in regards to that, and they are looking into it.
To reset your account login, i think you know what to do. If possible, get in touch with the same mod so they might know what's going on with your account.

In my opinion you are experiencing several different issues at the same time coincidentally they are not trying to get rid of you.
My legal opinion is that no company can deny an individual service without cause
