look to me 2 different plans. You sure your credit card not used for another account?
ask PM to trace. Open them a ticket. Steps above

I was charged on two separate dates. Once for my actual bill ($39.55). Also again the following day for a top up fee of $16.95. I should not be paying any top up fee. I am no where near my usage limits each month.

how you see it double charged? on the credit card statement?
if you see 2 lines on your PM's My Account's Transaction history, then it is not double charges. Every renewal shows 2 lines but actually just one charge. So, it will only be double charges if you see 2 lines on your credit card statement
if you really see 2 charges on the card monthly, ask PM to help by open proper ticket.
Just open ticket with PM using the Orange Chatbot icon on the lower right. (For 2FA when login, you might need to use email to receive if you cannot receive the text on the phone.) Type the question Submit ticket and select Contact Us to get to ticket open screen
