Speaking for myself, I called *611 when I needed the code.

If you became a customer before January 27, 2015, you can call *611 from your cell phone or 1-855-4Public from any telephone.

If the above does not apply to you, you must communicate with Public Mobile by web mail only. Click Contact Us in the lower column on this site's home page. Another page will open with Email Away on it. Click that. Another page will open with a web form and a hyperlink called None of the above. Click None of the above, and submit your request to Public Mobile.

To request a PUK code, you have to send an email to Public Mobile.

 

 

 

Click on this link : https://publicmobile.ca/en/on/contact-us

 

 

Then, at the bottom right of the page, click on "Email away" button.

  - Choose "

 

Then click on "Email Us" and fill up the form.

 

 

 

An agent from Public Mobile will answer you before the end of tuesday.
