https://www.ontario.ca/page/home-community-care says "To contact Ontario Health atHome:

call 310-2222 (no area code required)"

It doesn't work with Public Mobile. I've raised a ticket

click on that link you provided...click on Use Ontario Health at Home map / click on your area of Ont to see the toll free 800 or 888 number for local branch info.
