IMEI is obviously my phones, the PIN, is that for the old account, and as for alternate phone number, whose number is that for?

If at all possible, use only the account numnber but leave the IMEI field blank. In addition, never fill in the PIN for number porting (that option is really there as a last resort). Alternate phone number is a another phone number for Public Mobile to contact you if something goes wrong with the number port, but they almost never contact customers using the alternate phone number.

There's no such thing as an ESN or MEID any more. Those haven't been used since CDMA. Even the text message that Public Mobiel sends about porting failture incorrectly refers to MEID. If customers look specificly for MEIDs, they will never find them.
