If it has been activated, return to the place of purchase then pick one up at a Telus/Koodo store for $10 or from Amazon for $4.99.

You can only activate the SIM on the PM app.

All activations must be done online. Using the app is an online activation and requires an internet connection.

People might assuming that online mean on your computer or laptop. That is why I made the comment but it is a good point about the app being online.
