You will need to contact customer service agent to add the code.

Either submit a ticket for CSA help or directly private message CSA via the following link:

at Step 4, Review & Pay, there is a button to enter the friend referral code.

Just in case you missed that and already activated, you can open ticket with PM support and ask them to add it back for you

Contact customer support as per @Dunkman 's instructions. For future reference you add the referral code and any promocodes on the payment page. The lettering is tiny and easy to miss......
