A company, as par of their service procedure, need my phone bill statement on which my number, full name and address is shown. How can I get this statement?

Check the payment history from My Account https://myaccount.publicmobile.ca/en/account/payment/payment-history

you can download the invoices there and see if that is sufficient for what you need
