The default "SIM" PIN or unlock key is 1234. if you have changed it and do not remember it, that is when you need the PUK or "Personal Unlock Key" - this is not documented anywhere on the SIM card, but is an 8-digit number. Don't try to guess it or you will permanently "brick" your SIM card after too many tries. You will have to open a ticket with the CS agents to tell you what the code is to reset the SIM PIN, after providing them your SIM Card number.

The default "SIM" PIN or unlock key is 1234 ....

Beware that if this is indeed a locked SIM requesting PUK, you will likely be given a limited number of access attempts before the SIM permanently deactivates itself. Maybe three guesses, maybe ten, maybe a limited number per day or whatever. It apparently varies between device OEMs, specific phone models, and SIM providers.

It's hard to know exactly what is locked and what unlock password is being requested without some sort of screenshot or photograph or text descriptor of the "unlock" prompt. A phone and/or a SIM can be "locked" many different ways, there are many different prompt screens to "unlock" things.

@computergeek541 offered the correct solution for contacting PM and requesting a PUK, if it is indeed what is needed.
