All you need to do is login your account then go to your Profile and click on Report Lost/Stolen Phone.

This will cancel your SIM and blacklist your phone.

Doing the above will not blacklist the phone.

It also does not cancel the SIM card. The service is temporarily stopped. Declaring phone found will restore service using the same SIM card.
