I want to upgrade my iPhone SE to an IPhone 8 Plus. I believe I can just swap my sim from the old phone to the newer one. Is this correct?

Yes, you can just put your existing SIM card into the new phone. There should be options to have your Apple account data restored too.

As for transferring all settings/data from old to new phone, here is Apple Link with details.

The last thing you want to do is to set up everything all over, so use either device-to-device or device-to-itunes-to-device option.
