If you chose 3GB on 90 days plan then it is for the whole 90 days.

its 3gb data over the 3 months. So be wise with your data mangement! (90 day plan) So if you use all of it in the first month, you need to buy more data

What happens if I go over my data for the month? How much is the rate if I go over 200mb if I don't purchase the add on?

You can't go over. The data just stops working.
