Suite/Apartment (if applicable):
City:
Province:
Postal Code:
Customer phone number
Account #
Email address (if possible):
Requested Month / Period
Amount paid

Assuming you need monthly transaction history, you should be able to see past transaction information in your "My Transaction" tab, within your Account details.

This can also be found once you navigate to your Auto-pay page, if you already have that set up!
