Rewards available are Loyalty Reward, Autopay Reward, Community Reward and Refer-a-Friend Reward. When you log into My Account, you'll see whatever you're entitled to listed under My Rewards. What rewards are missing ?

If you feel that you were due rewards and they were missed...you should click the ? in lower right side of screen to start the process of moderator assistance. Type "contact a human" until you see "Contact Moderator".

Are the rewards automaticly applied or is it manually applied?

Rewards that you've earned are applied automatically to reduce any required plan payment.
