      <description>&lt;P&gt;You must&amp;nbsp;&lt;SPAN&gt;&amp;nbsp;provide&amp;nbsp; the account number and the name. Then leave the rest of of the fields blank. You&amp;nbsp; must chick the box that you are authorized to transfer the number.&lt;/SPAN&gt;&lt;/P&gt;&lt;P&gt;Let us know if that worked&lt;/P&gt;&lt;P&gt;Thanks&lt;/P&gt;</description>
