Yes, that is the only way you would be able to make a call from the US is to purchase the US roaming add-on

You can make calls to Canada or USA while in the USA with the roaming. Note: when people from the USA are trying to call your number while you are travelling the USA it may be long distance for them (they are calling to a Canadian number). Many USA mobile plans don't include Canada (like we don't include USA long distance except for Public's 50 dollar plan). Most USA phone plans will include international texting so while I am roaming in the USA I get my USA relatives to text me and then I call them back.
