Activate you PM card and pick a temporary number. Once everything is setup including your selfserve account. Just sign in to your account and choose the change number option and then you will be able to setup the transfer of you number by providing the requested information.

I just did this from Bell. Very easy. Either look at your old account online or phone your old provider. You'll need either your Account #, Phone Serial #, or one other number I forget. You just enter that along with your phone # you want to switch, and it does it for you. If you're with Rogers, Bell, etc. then your old plan will also automativally get cancelled (but I'd call them or look online to confirm)
