Can you look in all your email boxes for any emails from here? That would be your login email. Then you could try the Forgot your password? link.

By the subject line, I'm assuming you want to remove autopay and such so as to let the account lapse.

You could turn off autopay using the 611 service.

No such thing as cancelling account. You just stop paying.
