Long distance calls billing based on dialing form?

Taking into account https://productioncommunity.publicmobile.ca/t5/Discussions/If-I-text-or-call-a-local-number-while-they-re-out-of-the/m-p/75588/highlight/true#M19466 is my call always considered long distance if I call +1-area code or only if that code in another province/country?

Only to another province/country. Adding +1 to some provincial calls eliminates an annoying message that you may need long distance. With PM, any service that works is "free" because everything allowed is prepaid.
