$40 promotion plan, with ....

I want to get the $40 promotion, but need unlimited Canada/USA calling.

if I build my own, the promotion comes out to $45 (priced individually) and when I add USA calling, takes it to $50.

How can I get the promotion, and add USA calling (assuming it is $5/month?) for a total of $45 ?

It is not possible...

You'll have to choose between the promo + 400 minutes international add-on for $15 (comes down to less than 4¢/min and doesn't expire), or the $50 plan you are mentioning, depending on your usage.

I’ve had the 200+ USA and Canada wide extra for over a hear (two?) and I’m only at like 40 minutes used, it’s easy to top up, (I’ve done it once on the data addon) so it’s probably the best route unless you’re buying the addon more than once or twice a month.

its great knowing it’s not going to just expire on me.
